Mindset Coaching Agreement
Nature of Services. Inner Strength Transition provides mindset coaching, accountability coaching, and personal development support. These services are educational, motivational, and reflective in nature. They are non-clinical and non-medical.
Not Therapy or Medical Care. Lorenzo Chambers is not a licensed physician, psychologist, psychiatrist, licensed mental health counselor, therapist, or medical provider. Mindset coaching does not diagnose, treat, or cure any mental health condition. It is not a substitute for psychotherapy, medication, or crisis care. If you are in crisis or experiencing thoughts of harming yourself or others, call 988 (Suicide & Crisis Lifeline), 911, or go to your nearest emergency room.
No Guaranteed Results. Inner work is personal and unpredictable. Outcomes depend on your honesty, effort, and life circumstances. No specific emotional, financial, spiritual, or relational outcome is promised or guaranteed.
Confidentiality. Everything shared during coaching sessions is confidential, except where disclosure is required by law (for example, imminent risk of harm to self or others, or suspected abuse of a minor or vulnerable adult) or where you give written permission.
Client Responsibilities. You agree to (a) show up prepared and honest; (b) do the reflection work between sessions to the best of your ability; (c) continue to work with any licensed medical or mental health providers you already see; (d) consult a qualified professional before changing any medication, treatment, or medical protocol.
Cancellation & Rescheduling. Sessions cancelled with less than 24 hours notice may be forfeited. Repeated no-shows may result in termination of services without refund. Monthly coaching packages are non-refundable once the billing period begins.
Voluntary Participation. Your participation is entirely voluntary. You may pause or end coaching at any time in writing.
Governing Law. This agreement is governed by the laws of the State of Florida.
